Clinical Insights Into Bridge to Recovery

چکیده
History of Mechanical Circulatory Support Devices Advanced heart failure (HF) is a major cause of morbidity and mortality in the United States, and heart transplantation remains as the gold standard therapy. Because of scarcity of donor organs, the application of mechanical circulatory support devices has become a crucial approach in HF therapy as a bridge to transplantation. Therefore, mechanical circulatory support devices have existed both conceptually and experimentally for >40 years, along which an exponential evolution of mechanical circulatory support device technology has occurred. To mimic human physiology, the first generation of left ventricular assist devices (LVADs) were pulsatile volume displacement pumps (HeartMate XVE LVAS, Thoratec PVAD, and Novacor, etc). Insights demonstrating the inessentiality of the pulsatile nature of LVADs for survival from a physiological standpoint propelled the design of second-generation continuous flow devices (HeartMate II [Thoratec Inc], the Micromed DeBakey VAD, Berlin Heart Incor [Berlin Heart AG], and the Jarvik 2000 [Jarvik Heart Inc]), which emerged with superior safety and durability. Consequently, the HeartMate II was approved for bridge to transplantation and destination therapy in the United States. Recent reports have demonstrated a 2-year survival rate of 87% in destination therapy patients under intense surveillance, comparable with open heart transplantation survival statistics. In parallel, these promising outcomes of LVADs in HF therapy have spawned the translational research field of LV reverse remodeling, which has already shown great promise for elucidating underlying molecular and cellular mechanisms.

Newer Insights into the Prsevention of the Common Cold
Since most colds are self-limiting, symptoms will usually resolve on their own in seven to ten days. For the majority of patients, treatment is symptomatic and targeted at the most bothersome symptoms. Specific nonprescription therapy may relieve cold symptoms but should be used with caution, especially in at-risk patient populations. Patients should monitor for worsening symptoms, and progress...
متن کاملComputational insights into fluconazole resistance by the suspected mutations in lanosterol 14α-demethylase (Erg11p) of Candida albicans
Mutations in the ergosterol biosynthesis gene 11 (ERG11) of Candida albicans have been frequently reported in fluconazole-resistant clinical isolates. Exploring the mutations and their effect could provide new insights into the underlying mechanism of fluconazole resistance. Erg11p_Threonine285Alanine (Erg11p_THR285ALA), Erg11p_Leucine321Phenylalanine (Erg11p_LEU321PHE) and Erg11p_Serine457Pro...

Although the nerve auto graft still remains the clinical Gold standard in repairing nerve injury gaps, many advances have been achieved to guide regenerating axons across the lesion. Functional recovery after peripheral nerve lesion is depended upon accurate regeneration of axons to their original target tissues. To increase the prospects of axonal regeneration and functional recovery, research...
متن کاملA Case of Gastric Ulcer With Mucosal Bridge
Endoscopic images of the mucosal bridge with gastric ulcer are unusual. The mucosal bridge is presumed to have changed to a special form based on repeated development of gastric ulcer. The clinical course of mucosal bridge is unclear.
متن کاملCharacterization of Liquid Bridge in Gas/Oil Gravity Drainage in Fractured Reservoirs
Gravity drainage is the main mechanism which controls the oil recovery from fractured reservoirs in both gas-cap drive and gas injection processes. The liquid bridge formed between two adjacent matrix blocks is responsible for capillary continuity phenomenon. The accurate determination of gas-liquid interface profile of liquid bridge is crucial to predict fracture capillary pressure precisely. ...
متن کاملPercutaneous Ventricular Assist Devices and ECMO in the Management of Acute Decompensated Heart Failure
The successful treatment of acute decompensated heart failure continues to evolve with an increasing utilization of nondurable mechanical support devices. Indications for acute support have broadened to include their use as a bridge to recovery or decision (for durable ventricular assist devices [VADs] or heart transplant). Available devices have improved in terms of effectiveness, ease of inse...
